Pediatric moyamoya MRI score: an imaging-based scale to predict outcomes in surgically treated pediatric patients with moyamoya

OBJECTIVE Moyamoya is a progressive arteriopathy that predisposes patients to stroke due stenosis of the intracranial internal carotid arteries and their proximal branches. Despite morbidity caused by this condition, ability accurately predict prognosis for individual remains challenging. The goal study was develop systematic scoring method based on parenchymal findings preoperative brain MRI long-term outcomes surgically treated pediatric with moyamoya. METHODS A retrospective surgical cohort (≤ 18 years age at time initial surgery) moyamoya from single center were studied. Radiological variables existing correlations between in or other vascular diseases chosen score easily defined could be rapidly assessed used make numeric score. Calculated scores correlated clinical outcome measures using Pearson correlation coefficient area under receiver operating characteristic curve (AUROC). RESULTS total 35 children disease syndrome included study, median follow-up 2.6 surgery. (PMMS) consists ischemic changes (0–2; 0 = none, 1 focal, 2 diffuse), encephalomalacia hemorrhage (0–1; not present, present). PMMSs highly modified Rankin Scale last (r 0.7, 95% CI 0.44–0.84; p < 0.001) as six-point scale, when dichotomized (AUROC 0.85). CONCLUSIONS PMMS found simple tool data quickly calculated disability. This may aid future development predictive models syndrome.
